Why a health check? Most PT insurers expect a pre-exercise health questionnaire and signed consent before the first session. The form gives you both on paper.

Can I track progress? Yes. The goal setting sheet records starting measurements and a recheck date, and the session log keeps every workout in one place.

Can I add my own questions? Yes. Every line is editable, so you can add questions or your insurer's wording.
