Tattoo Artist Client Record Book | Printable + Tablet PDF
Keep every client's details, patch tests and treatment notes in one tidy place. This tattoo client record book comes as two PDFs: a printable A4 version for a ring binder, and a linked tablet version for GoodNotes, Notability, Xodo or Samsung Notes.
What's inside
Client pages (one set per client):
- Client profile and ID: contact details, an 18+ photo ID checklist (passport, driving licence, PASS card), style notes and consent
- Consent and medical: a yes/no checklist of medical questions to go through before each session, plus client confirmations and signature
- Design and placement: front and back body outlines to mark placement, design, size, style, stencil approval, deposit and quote, plus a dot-grid sketch area
- Session log: time, area, needles, and inks with brand, colour and batch number for every session
- Aftercare and touch-ups: aftercare given, healing check-ins and a touch-up record
Plus: a how-to-use page, a plain-English UK GDPR page on keeping client records safe, a client index and notes pages.
Two versions
- Printable PDF (11 A4 pages): print the client set as many times as you need. A wide left margin leaves room for hole punching.
- Tablet PDF (294 pages): a contents page and A to Z tabs on every page. Each letter has a divider and room for two clients, and every page links back to contents.
Who it's for
Tattoo artists and studios who want proper records without paying for software.

Questions
Which apps does the tablet version work in?
Any PDF note app that keeps links, such as GoodNotes, Notability, Xodo or Samsung Notes. If a link does nothing, switch to read mode or tap with your finger instead of the pen.
What if I have more than two clients for a letter?
Most note apps let you duplicate pages, so copy a client set and add it after that letter. On paper, just print another set.
Does it replace my consent form?
No. It is a checklist and record to go through with your client. Use your own consent form and any wording your council licence or registration asks for.
Good to know
- These are blank forms and record sheets. They are not legal, medical or insurance advice, and they do not replace your insurer's rules or your product instructions.
- The GDPR page is a friendly starting point, not legal advice. See ico.org.uk for the full guidance.
- Copied pages in a tablet app keep links to the original pages, so use the tabs to get around.
- For your own business use. Please do not resell or share the files.
